Head Start Program Details, hours, photos, information: Ann Klose First School

Please note:

School District Residency and Registration is required for 4 year old UPK classes.

Bring the completed application with all of the supporting documents listed below to the nearest Head Start location listed on page 2.

Copies of your documents will be made and you will be given a receipt.

They will not accept incomplete applications.

Be sure to bring:

• ChilG V Original
• Birth Certificate
• KLOG V Current Immunization Record
• KLOG V Health Insurance
• Medicaid Card (If child has coverage)
• Proof of Income (W-2 from last year, Public Assistance Budget Sheet, 4 consecutive weeks worth of pay stubs, Unemployment Documentation, Child Support Payments, SSI Grant Letter, Foster Care Grant, Tax Form 1040)

Court Papers Verifying Guardianship (if applicable) You will notice that an ACAP Head Start Physical Exam Form is attached.
