You will need to complete an application and submit the following:

• Proof of IncomeBirth Certificate
• Immunization Record
• Proof of ResidencyInsurance/Medical Card
• Social Security Card

The Nicholas County Head Start Program will be accepting applications on a continual, ongoing basis.
