Head Start Program Details, hours, photos, information: Chisholm Head Start

Mid florida community services, inc. operates Head Start centers for low-income eligible families in Hernando, Sumter and Volusia County.
To apply you can call your local center to make an appointment or call
352-754-2464(Hernando County)352-568-0821(Sumter County) 386-736-1325(Volusia County). Please bring with you the following information to your application appointment:
1. Proof of child's age (such as Birth Certificate, immunization record); Proof of residency such as: Florida driver's license, utility bill, I.D., etc.
2. Proof of income for parent/guardian for the last 12 months or previous calendar year
3. If child has a disability:
IEP, diagnosis etc.4. Proof of custody (if applicable).
