Child Enrollment Application RequirementsWhen applying please bring the following...

1. Certified Birth Certificate

2. Total household income for past 12 months - please bring one or more of the following as applicable:

A. W-2 forms

B. Check stubs

C. Tax return

D. Letter of Employment

3. Insurance Card

4. Shot record for each child to be enrolled ** If applicable, include Work First Family Assistance Statement and Medicaid number
