Necessary Documentation for Enrollment:

• Childs birth certificate
• Proof of Income (last years Income tax return or previous 4 qtr. Earnings report from Texas Workforce Commission)
• Immunization Record
• Insurance Card (Private Insurance, Chips, Medicaid card)
• Proof of residency (utility bill with parents name on it)
