Checklist for Enrollment-Proof of Childs Age: Birth certificate, Passport/Visa, Medical card, or MO Heathnet card-Proof of Income: Tax return, W-2, SSI award letter, TANF award letter, Check stubs Unemployment letter, Child support payment, and Foster Care subsidy-Health Information: Current physical exam/well-baby exam records, current immunizations, dental exam and blood work results
All children enrolled in the program are required to have a health screen that includes the following:-Physical exam-Health history-Hearing-Lead screen-Sickle cell test-Blood pressure-Hemoglobin screen-Growth assessment-Vision-Nutrition assessment-Dental exam-Immunizations must be complete or up-to-date
