Enrollment ProcedureYou MUST include photocopies of the following:A copy of your child's birth certificateor other acceptable form of legal identification that includes your child's date of birth.Proof of income for the past 12 months.This can be W2, Federal Income Tax form 1040, check stubs, award letters from DSHS/SSI or other documentation.Copies of your child's immunization records.Federal/State law requires children must provide proof they are current on immunizations or have signed a waiver before they can attend preschool.Please download and print and application form (spanish form) and return it to our Main Office (by mail or hand delivered).
